Patients' perspectives of care management: a qualitative study

Working as Health Revenue Cycle Management Consultant, has many advantages. Including but not limited to improve patient journey and understanding of patient perspective towards care management. This study has emphasized on the importance to listen to patient experience with care management as the following:

Care managers are staff who works closely with high-risk patients and primary care teams to:

  1. Taught patients about their conditions and how to manage them
  2. Track patients’ chronic condition management over time
  3. Follow up with patients after an emergency department visit or hospital discharge
  4. Monitor medications
  5. Facilitate communication between the patients and the primary care physicians
  6. Help patients get access to community support services
  7. Provide emotional support
  8. Navigate the healthcare system and community-based supports

Their tasks and responsibilities are most effective if they:

  1. Work within a primary care practice,
  2. Meet patients in person,
  3. Communicate with them by phone between visits,
  4. Coordinate with their primary care physician.

The purpose of care managers is:

  1. Reduce costs
  2. Improve primary care delivery,
  3. Improve patients’ and providers’ experiences, and health outcomes

They concluded, that many of patient have seen the advantages of having someone to manage / control, monitor and follow up with them; but not forgetting that there are many areas of improvement that can be done to increase rate of patient satisfaction.
